Clinical Assessment and Eligibility Registered Nurse
This is a full-time position, 35 hours per week, M-F.
SeniorCare, Inc. is seeking a compassionate and experienced Registered Nurse to join our team as a Clinical Assessment and Eligibility RN. In this role, you will support older adults and consumers across community and nursing facility settings by assessing clinical eligibility, coordinating person-centered care, and helping ensure safe, cost-effective service planning.
Key Responsibilities:
- Assess consumers for clinical eligibility for Medicaid, state home care programs, and nursing facility services.
- Partner with Care Managers and interdisciplinary teams to develop and review person-centered service plans.
- Determine needs for personal care, home health aide, and other supportive services.
- Support safe transitions between nursing facilities and community settings.
- Maintain strong documentation, regulatory compliance, and communication with providers and community partners.
- Participate in peer audits, team meetings, and ongoing quality improvement activities.
Requirements:
- Current Massachusetts Registered Nurse license required.
- A diploma in Registered Nursing (RN) with at least 2 additional years of direct service experience in community-based nursing or long-term care (LTC), or
- An associate’s or bachelor’s degree in nursing with at least 1 year of clinical experience in a community or LTC setting.
- Strong leadership, organization, interpersonal communication, and documentation skills.
- Working knowledge of community resources, reimbursement systems, and intermediate computer skills.

Starting range: $33.00 - $36.00 per hour. The range for this position is established based on internal equity and market data. Individual rates may vary within the established range based on directly relevant experience and credentials. Final offers are not negotiable.
