As a Community Support Specialist, you will provide intensive, short-term, community-based behavioral health services that empower adults living with mental health conditions, substance use disorders, and co-occurring disorders to achieve stability, improve functioning, and successfully navigate healthcare and community support systems.

As a Community Support Specialist, you will:
- Conduct comprehensive needs assessments and develop individualized, person-centered service plans.
- Provide assertive outreach and engagement in homes, hospitals, shelters, correctional settings, outpatient clinics, and community locations.
- Coordinate behavioral health, primary care, psychiatric, housing, vocational, educational, and social service supports.
- Assist individuals in obtaining MassHealth benefits, transportation, food resources, financial assistance, employment resources, and other community supports.
- Collaborate with primary care providers, hospitals, CBHCs, Mobile Crisis Intervention teams, therapists, psychiatrists, community agencies, family members, and other collateral contacts.
- Assist members with appointment scheduling and follow-through.
- Support members in identifying and overcoming barriers to treatment and recovery.
- Develop crisis prevention and safety plans.
- Monitor progress toward measurable treatment goals.
- Maintain accurate, timely, and compliant documentation in the electronic health record.
- Participate in supervision, interdisciplinary treatment meetings, and quality improvement activities.
Minimum Qualifications Required
- High School diploma
- Bachelor's degree in:
- Social Work
- Psychology
- Counseling
- Human Services
- Sociology
- Criminal Justice
- Behavioral Health
- or another related field
OR
- Two years of relevant behavioral health, human services, and community support experience or lived experience of homelessness, behavioral health conditions, and/or justice involvement.
- Valid driver's license
- Reliable transportation
- Ability to travel throughout the community
Preferred Qualifications
- LSW

- LMHC
- LADC
- CADC
- Recovery Coach Certification
- Experience working with adults living with serious mental illness, substance use disorders, or co-occurring disorders.
- Familiarity with MassHealth services.
- Knowledge of trauma-informed care.
- Motivational Interviewing experience.

Physical Requirements
- Frequent travel throughout the community.
- Must be capable of meeting the community support needs of clients with behavioral health issues placing them at risk of requiring 24-hour mental health or substance use disorder treatment.
- Ability to sit, stand, walk, and drive for extended periods.
- Ability to safely perform home and community visits.
