POSITION DESCRIPTION
TITLE: Therapist SALARY RANGE: $28 - $40/hour
CATEGORY: Full-Time REPORTS TO: VP Clinical Services/Medical Director
SUMMARY:
This position is part of the clinical team and provides outpatient mental health and/or substance use disorder counseling services to children, adolescents, adults, couples, and families. The Therapist will work collaboratively with the CORE team, referral sources, community partners, and other healthcare providers to support client wellness, stability, and long-term recovery. Responsibilities include conducting assessments, developing treatment plans, providing individual, group, couples, and family therapy, maintaining clinical documentation, and coordinating care as needed. This position may include a combination of in-office and telehealth services, with flexibility for some remote work based on organizational and client needs. This position requires Afterhours coverage on a rotation with other Outpatient Therapists.
ESSENTIAL DUTIES:

Maintain strict confidentiality and comply with HIPAA and 42 CFR Part 2 requirements.

Conduct clinical assessments, screenings, intake evaluations, and treatment planning.

Provide individual, group, couples, and family therapy services.

Provide mental health and substance use disorder counseling services to children, adolescents, and adults.

Develop, implement, monitor, and update individualized treatment plans based on client needs and progress.

Complete clinical documentation, progress notes, treatment plans, assessments, discharge summaries, and other required documentation within established timelines.

Maintain accurate and timely documentation in the electronic health record.

Coordinate care with physicians, psychiatrists, case managers, schools, probation officers, social service agencies, referral sources, and other community providers as appropriate.

Participate in multidisciplinary staffing and case consultation meetings.

Provide crisis intervention and risk assessment services as needed during work hours.

Complete suicide risk assessments and safety planning when clinically indicated.

Facilitate psychoeducational and therapeutic groups utilizing evidence-based practices.

Utilize evidence-based treatment modalities including Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), trauma-informed care, relapse prevention, and other clinically appropriate interventions.

Assist clients with discharge planning, referrals, aftercare planning, and connection to community resources.

Monitor client progress and modify treatment interventions as clinically appropriate.

Maintain productivity expectations and scheduling requirements established by the organization.

Participate in clinical supervision, staff meetings, trainings, and continuing education activities.

Maintain licensure, certifications, and continuing education requirements as required by the Wisconsin Department of Safety and Professional Services and applicable licensing boards.

Collaborate with insurance companies and managed care organizations regarding authorizations, reviews, and continuation of care when necessary.

Provide telehealth services in accordance with organizational policies and state/federal regulations, when applicable.

Participate in after-hours on-call rotation, if assigned.

Required: Licensed as an LCSW, APSW, LPC, LPC-IT or MFT
Preferred: Master’s degree in counseling, Psychology, Social Work or related field
Excellent communication skills – verbal and written
Computer proficiency
Must be able to work independently
Must have good working knowledge of DHS 75, 94.
