Shift: Monday-Thursday 8:30am-4:30pm, F 8:00am-4:00pm
Salary: $57,159.44

Lifeworks is hiring a full time Autism Intensive Supports Coordinator to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve.
Essential Duties:
- Provide clinical support services to individuals with autism.
- Meet with a caseload on a scheduled basis (weekly, bi-weekly), including home visits and community locations to provide services.
- Provide services in a way that recognizes and respects individuals’ ethnic and cultural diversity.
- Provide case management for individuals, including coordination with other agencies and providers.
- Review cases and collaborate with Lifeworks Clinical Team on a regular basis for support and guidance.
- Coordinate with DDS Autism Service Coordinators and providers to ensure a team approach to services
- Assess new referrals and establish person-centered goals.
- Help individuals understand and access services, benefits and resources available to them, including Mental Health services.
- Assist the individual in utilizing existing services or obtaining new supports as needed.
- Provide and/or procure crisis intervention as needed.
- Report on metrics established by DDS.
- Ability to work a flexible schedule to meet the needs of individuals and facilitate programs
- Excellent interpersonal skills.
- Ability to multitask and prioritize tasks with attention to detail.
- Ability to work in a team environment.
Requirements:
- Experience working with Individuals with ID/DD
- Associates Degree plus 5 years of work experience in the Human Services field.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License required
- Excellent verbal and written communication skills
