PRN RN or LPN – In-Home Assessment & Care Coordinator
Experienced Home Care Nurse Wanted
Home Care Plus is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team on a PRN basis to conduct in-home client assessments, reassessments, and care coordination throughout the Charleston and Mount Pleasant area.
This is not a bedside nursing position and does not involve providing skilled nursing care in the home.
This position is ideal for an experienced nurse who understands private-duty/non-medical home care, senior care, aging in place, and the importance of accurately identifying a client's needs in the home environment.

We are specifically looking for a nurse with relevant industry experience.
To be considered, candidates should have previous experience in private-duty home care, non-medical home care, home health, hospice, senior care, geriatric care, case management, care coordination, or another closely related setting.
Strong preference will be given to RNs/LPNs who have previously completed in-home assessments for a non-medical or private-duty home care agency.
This is not an entry-level nursing position.
What You'll Do
As our PRN Assessment Nurse, your primary responsibility will be evaluating clients in their homes and helping ensure we have an accurate, individualized plan for providing safe and appropriate non-medical care.
Responsibilities include:
- Conduct initial in-home client assessments
- Complete client reassessments as required or when care needs change
- Assess activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Evaluate mobility, fall risk, cognition, personal care needs, nutrition, safety concerns, environmental factors, routines, and available family support
- Identify the appropriate level of non-medical caregiver assistance
- Develop and update individualized plans of care
- Communicate assessment findings and recommendations clearly to clients, families, and the Home Care Plus team
- Identify changes in condition or circumstances that may require additional attention or referral to an appropriate healthcare professional
- Coordinate care needs with clients, families, caregivers, and other providers when appropriate
- Help ensure caregivers have clear, accurate instructions regarding each client's needs and routines
- Complete assessment documentation accurately and promptly
- Participate in care conferences when needed
- Maintain appropriate confidentiality and professional boundaries
- Recognize when a client's needs may exceed the scope of non-medical home care and appropriately escalate concerns
Required Qualifications
- Active, unrestricted South Carolina RN or LPN license
- Previous professional experience working with older adults
- Experience in home care, home health, hospice, senior care, geriatrics, case management, care coordination, rehabilitation, or a closely related healthcare setting
- Strong assessment and documentation skills
- Understanding of ADLs, IADLs, fall risk, mobility, cognitive changes, dementia-related needs, and home safety
- Ability to communicate professionally and compassionately with seniors and their families
- Strong clinical judgment while understanding the boundaries of non-medical home care
- Ability to work independently in clients' homes
- Reliable transportation and ability to travel throughout our service area
- Excellent organization, time-management, and follow-through
Strongly Preferred
- Previous experience completing in-home assessments for a private-duty or non-medical home care agency
- 5+ years of nursing experience
- Geriatric nursing experience
- Experience developing or updating client plans of care
- Home care admissions or assessment experience
- Hospice, case management, care coordination, or discharge-planning experience
- Experience working with clients with dementia, Parkinson's disease, COPD, mobility limitations, or other age-related conditions
- Familiarity with home care software or electronic documentation systems
Who Will Be Successful in This Role?
We're looking for someone who can walk into a client's home and see the whole picture—not just a diagnosis.
You notice whether someone is struggling to get out of a chair safely. You ask about recent falls. You look at how they're managing meals, medications, bathing, dressing, mobility, and everyday routines. You listen to what the client says and what the family is concerned about.
Most importantly, you understand the difference between skilled nursing needs and non-medical home care needs and can help us determine how Home Care Plus can safely support the client.

Schedule
This is a PRN position based on client assessment and reassessment needs.
The ideal candidate will have flexibility to accommodate assessments as new clients begin services or when an existing client's care needs change.
This position may be a great fit for an experienced RN or LPN who wants to remain involved in senior care while enjoying the flexibility of PRN work.
