Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
- Support patients with specialized care needs such as catheter care, hospice care, dementia care, Alzheimer's care, and memory care.
- Maintain detailed medical documentation ensuring compliance with HIPAA regulations and confidentiality standards.
- Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients.
- Support patients during emergencies by knowing CPR procedures and responding swiftly to urgent situations.
- Collaborate with healthcare professionals to develop and implement personalized care plans tailored to each patient’s condition.
- Work effectively in home & community care settings or assisted living facilities to deliver consistent quality care.
- Must have current CHHA license.
Experience
- Prior experience in home health or caregiving roles is highly preferred; experience in nursing homes or long-term care environments is advantageous.
- Demonstrated ability to work with people with developmental disabilities or cognitive impairments such as dementia or Alzheimer’s disease.
- Familiarity with medical terminology and medical documentation practices is essential for accurate record keeping.
- Proven competency in patient observation skills and working with EMR/EHR systems for documentation purposes.
- Certification in CPR is required; additional certifications such as hospice care training or catheter care are a plus.
- Experience working with seniors or individuals requiring memory or dementia-specific care enhances your suitability for this role.
