Pay: $32.50–$36.00/hour (base rate; shift diffs & incentive pay available)
Schedule: Full-Time (3–11 Shift)
Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Apple Rehab Middletown, a trusted and family-owned rehabilitation and skilled nursing network dedicated to exceptional resident care. As an LPN Charge Nurse, you will play a vital role in coordinating high-quality nursing care for our residents while maintaining compliance with facility policies, safety standards, and state/federal regulations.
Key Responsibilities:
- Provide compassionate, professional nursing care to residents
- Administer medications and treatments safely and accurately
- Maintain clear, thorough documentation within regulatory guidelines
- Communicate effectively with families, physicians, and interdisciplinary staff
- Support a positive and collaborative team environment
- Ensure all care aligns with resident needs and medical best practices
Requirements:
- Valid Connecticut LPN license (in good standing)
- Knowledge of long-term care practices and state regulations
- Strong organizational and multitasking abilities
- Excellent communication and teamwork skills
- Long-term care experience preferred but not required
Additional Expectations:
- Weekend on-call participation required (compensation available)
- Per diem staff must work at least two weekend shifts per month
