Responsibilities
- Supervise and coordinate nursing staff to ensure high-quality patient care across all shifts.
- Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform individualized care plans.
- Administer medications, including injections and IV infusions, following strict protocols and documentation standards.
- Oversee medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems, ensuring accuracy and HIPAA compliance.
- Manage complex cases involving Alzheimer's care, dementia, memory care, and behavioral health needs with sensitivity and professionalism.
- Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies.
- Implement infection control procedures and maintain sterile techniques during procedures such as catheterization or specimen collection.
- Provide training and mentorship to nursing staff on topics like ICD coding, wound care, pain management, and emergency response protocols.
Qualifications
- Valid LVN license in the state of practice with current CPR/BLS certification.
- Critical care experience preferred, including ICU or emergency medicine settings such as Level I or Level II trauma centers.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with medical documentation is essential.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to geriatrics and long-term care.
- Experience working with individuals with disabilities, including developmental disabilities or those requiring hospice or palliative care.
- Familiarity with advanced procedures such as IV insertion/infusion, ventilator management, tube feeding, or spinal tap is advantageous.
- Excellent patient assessment skills coupled with the ability to triage effectively in urgent situations like acute pain or infectious disease outbreaks.
