JOB TITLE: ASSISTANT TEAM DIRECTOR
REPORTS TO: Team Director/PCC
JOB SUMMARY: The Assistant Team Director is a field-based clinical leadership position responsible for supporting the Team Director in the daily oversight, supervision, mentoring, and professional development of the interdisciplinary team, including Registered Nurses, Social Workers, and Spiritual Care Counselors.
The Assistant Team Director serves as a clinical resource and coach, conducting routine field visits to ensure the delivery of high-quality hospice care that is consistent with Hospice of New York standards, regulatory requirements, and best practices. This position provides direct observation, supervision, mentoring, and competency development for clinical staff while assisting with quality assurance activities, clinical record audits, documentation improvement initiatives, and performance monitoring.
The Assistant Team Director partners with the Team Director to promote excellence in patient care, staff engagement, interdisciplinary collaboration, regulatory compliance, and clinical outcomes.
RESPONSIBILITIES:
Clinical Leadership and Field Supervision
- Conduct routine field supervisory visits with Registered Nurses, Social Workers, and Spiritual Care Counselors.
- Perform joint visits with staff to assess clinical practice, communication skills, documentation practices, and adherence to hospice standards of care.
- Provide real-time coaching, mentoring, and education to staff in the field.
- Complete Back-to-the-Bedside (B2B) visits and leadership rounds as assigned.
- Serve as a resource to staff regarding symptom management, care planning, end-of-life care, psychosocial support, and spiritual care interventions.
- Monitor staff productivity, visit quality, and patient outcomes in collaboration with the Team Director.
- Assist in identifying performance improvement opportunities and creating individualized development plans.
- Support staff retention efforts through mentoring, coaching, recognition, and professional development activities.
Clinical Documentation and Quality Oversight
- Conduct ongoing clinical record reviews and documentation audits to ensure compliance with CMS Conditions of Participation, New York State regulations, accreditation standards, and Hospice of New York policies.
- Review comprehensive assessments, plans of care, visit notes, recertification documentation, hospice eligibility documentation, and interdisciplinary documentation for accuracy and timeliness.
- Identify documentation deficiencies and develop corrective action plans when needed.
- Provide education and feedback to staff regarding documentation standards and regulatory requirements.
- Monitor compliance with documentation timeframes and productivity expectations.
- Assist with chart audits related to Quality Assurance and Performance Improvement (QAPI) initiatives.
- Collaborate with Education and Quality Departments to develop strategies that enhance documentation quality and clinical outcomes.
Interdisciplinary Team Support
- Participate in interdisciplinary team meetings and assist in facilitating discussions related to patient care planning and clinical concerns.
- Ensure that patient care plans reflect interdisciplinary involvement and individualized patient goals.
- Promote effective communication among nurses, social workers, spiritual care counselors, physicians, aides, volunteers, and administrative staff.
- Assist with coordination of care transitions, inpatient admissions, General Inpatient (GIP) level of care, respite care, and discharges.
- Help resolve clinical concerns, patient complaints, and family concerns in collaboration with the Team Director.
Staff Development and Education
- Assist in orientation and onboarding of new interdisciplinary team members.
- Participate in competency evaluations and annual performance assessments.
- Identify educational needs and coordinate training opportunities with the Education Department.
- Support clinical competency validation through field observations and skills assessments.
- Promote adherence to evidence-based hospice practices and organizational standards.
Regulatory and Compliance Responsibilities
- Ensure compliance with all federal, state, local, and organizational regulations governing hospice services.
- Participate in QAPI activities, surveys, audits, and regulatory readiness initiatives.
- Monitor compliance with infection prevention, patient safety, emergency preparedness, and risk management requirements.
- Assist in implementing corrective actions resulting from audits, surveys, or quality reviews.
Operational Support
- Assist the Team Director with daily clinical operations and workflow management.
- Support scheduling coordination and coverage planning as needed.
- Assist in recruitment, interviewing, and selection of clinical personnel.
- Participate in committee work, special projects, and organizational initiatives.
- Serve as acting Team Director during periods of absence as assigned.
QUALIFICATIONS:
Required
- Current Registered Nurse (RN) license in the State of New York.
- Bachelor's Degree in Nursing (BSN) required; Master's degree preferred.
- Minimum three (3) years hospice experience.
- Minimum two (2) years of clinical leadership, supervisory, preceptor, or mentoring experience.
- Demonstrated knowledge of hospice regulations, Conditions of Participation, documentation requirements, and interdisciplinary care planning.
- Strong communication, organizational, coaching, and leadership skills.
- Proficiency with electronic medical record systems and clinical documentation review.
Preferred
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Experience conducting quality reviews, clinical audits, and performance improvement initiatives.
- Experience supervising interdisciplinary hospice teams.
Additional Requirements
- Ability to travel throughout Hospice of New York's service area, including Bronx, Manhattan, Brooklyn, Queens, and Nassau County.
- Reliable transportation, valid driver's license, and automobile insurance as required by New York State law.
- Ability to conduct frequent field visits and home, facility, and hospital rounds.
WORK ENVIRONMENT
This position is primarily field-based, requiring regular travel to patient homes, nursing facilities, assisted living residences, hospitals, and other care settings. The Assistant Team Director is expected to maintain a visible leadership presence in the field and provide direct support to staff where patient care is delivered.
The Assistant Team Director is intentionally structured as a field leadership and quality oversight role, with approximately:

This structure emphasizes direct observation of staff practice, mentoring of clinicians, and improvement of documentation quality and patient care outcomes.
Job Type: Full-time
Pay: $110,000.00 - $125,000.00 per year
