Overview
We are seeking a dedicated and energetic PHC Supervisor to lead and coordinate healthcare services within our community‑based facilities. This vital role involves overseeing patient care operations, supervising healthcare staff, and ensuring compliance with healthcare standards and regulations
Duties
- PHC Service Planning — Develop and implement individualized PHC service plans that support patient needs and regulatory requirements.
- Staff Supervision — Oversee unlicensed personnel, ensuring proper training, performance, and adherence to care standards.
- Care Coordination — Monitor daily patient care operations to maintain safety, quality, and continuity of services.
- Documentation Management — Ensure accurate, timely documentation and maintain compliance with communication and reporting protocols.
- Regulatory Compliance — Uphold state, federal, and organizational healthcare regulations and quality benchmarks.
- Team Collaboration — Work closely with interdisciplinary teams to support holistic patient care and operational efficiency.
Qualifications
- Education Requirements — Completion of two years of full‑time study at an accredited college or university.
- Equivalent supervisory experience in a healthcare or community‑based setting may substitute year‑for‑year.
- Supervisory Competence — Ability to perform all essential supervisory duties, including planning service plans, overseeing unlicensed personnel, and managing documentation and communication workflows.
- Healthcare Experience — Background in senior care, long‑term care, home health, or related environments strongly preferred.
Physical & Environmental Requirements
- Physical Demands — Regular use of hands, standing, sitting, and lifting 10–25 lbs as needed.
- Work Environment — Ability to work effectively in a moderate‑noise, fast‑paced healthcare setting.
- Task Readiness — Must meet all physical and environmental demands necessary to perform essential job functions.
Pay: $17.00 - $18.00 per hour
