POSITION SUMMARY: The Pain Program Coordinator oversees the day-to-day flow of patient care in the pain management program within the healthcare clinic. The Program Coordinator will work closely with healthcare providers, patients, and administrative staff to ensure the delivery of high-quality care to individuals with ongoing pain conditions. This position requires a strong understanding of pain management principles, excellent communication skills, and the ability to coordinate multiple aspects of patient care effectively.

Patient Care Coordination: Serve as the primary point of contact for patients seeking pain management services. Schedule appointments, coordinate referrals, and assist patients in navigating the healthcare system. Provide support and guidance to patients throughout their treatment journey.

Treatment Plan Management: Collaborate with healthcare providers to develop individualized treatment plans for patients with pain conditions. Ensure that treatment plans align with evidence based practices and patient preferences. Monitor patient progress and adjust treatment plans as needed.

Medication Management: Coordinate medication management services for patients receiving pharmacological treatment for pain. Facilitate prescription refills, medication reconciliation, and medication education sessions. Work closely with providers to optimize medication regimens and minimize risks.

Multidisciplinary Team Collaboration: Facilitate communication and collaboration among members of the pain management team, including physicians, nurses, physical therapists, psychologists, and other healthcare professionals. Coordinate interdisciplinary meetings to discuss patient cases and develop comprehensive care plans.

Patient Education: Provide education to patients and their families about pain management strategies, treatment options, and selfcare techniques. Offer resources and support to help patients better understand their condition and manage their symptoms effectively.

Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the delivery of pain management services. Collect and analyze data related to patient outcomes, satisfaction, and adherence to treatment protocols. Identify areas for improvement and implement strategies to optimize program effectiveness.

Documentation and Reporting: Maintain accurate and uptodate documentation of patient encounters, treatment plans, and communications within the electronic medical record system. Generate reports and summaries as needed for clinical and administrative purposes.

Community Outreach: Engage in community outreach activities to raise awareness about the pain management program and its services. Foster partnerships with local organizations, support groups, and healthcare providers to enhance access to care for individuals with chronic pain.

Licensed nurse (LVN/RN) required. Bachelor's degree in Nursing, preferred.

Current CA licensure as a Licensed Vocational Nurse or Registered Nurse.

Experience working in pain management or a related clinical setting preferred.

Strong knowledge of pain assessment, treatment modalities, and best practices in pain management.

Excellent interpersonal and communication skills, with the ability to interact effectively with patients, families, and healthcare professionals.

Proficiency in electronic medical record systems and Microsoft Office Suite.

Ability to prioritize tasks, manage time efficiently, and work independently as well as part of a team.

Reaching / Handling: Ability to input information into computer systems and retrieve and work with appropriate paperwork, equipment, and supplies. Use of standard office equipment, including computers, telephones, calculators, copiers, and faxes. Work is performed in an office/clinic environment; continuous contact with other staff and the public.
