This travel assignment places you on a LTC unit within a skilled nursing environment. You will work with a team to provide direct patient care for residents, participate in interdisciplinary rounds, and support the daily living activities of aging adults. The assignment runs for 13 weeks with a set start date and a mid-to-late fall end date, and it requires adherence to unit policies and facility procedures.

Responsibilities
- Deliver direct nursing care to LTC residents in the designated unit, based on individualized care plans.
- Administer medications and treatments as ordered; document administration and any changes per protocol.
- Monitor residents’ vital signs, symptoms, and overall condition; identify and report changes promptly to the supervising clinician.
- Assist with activities of daily living (ADLs) and mobility, promoting safety, comfort, and dignity.
- Maintain accurate documentation in PointClickCare EMR; communicate effectively with the interdisciplinary care team; participate in care rounds and handoffs.
- Provide nursing support with dementia/memory care and orthopedic-related needs, as well as other common LTC diagnoses on the unit; float within the same building as needed.
Requirements
- Active LPN license in New York; Nursys license verification with an ACTIVE NY license and all active licenses
- Current BLS certification
- 1+ year of experience in a long-term care (LTC) facility
- EMR experience: PointClickCare
- Education: as required for LPN licensure (documented in application materials)
- Driving license with address matching demographics
- RTO (time off) must be included at time of submission
- Ability to work on the LTC unit and float within the same building as assigned
