Job Summary:
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide quality nursing care in along-term care and rehabilitation setting. The LPN will support residents’ medical needs, ensure their comfort and safety, and work closely with the healthcare team to deliver excellent patient care.
Responsibilities:
- Administer medications and treatments as prescribed
- Monitor residents’ health status and report any changes to the RN or physician
- Assist in implementing individualized care plans
- Provide basic nursing care and support residents with daily needs
- Maintain accurate and timely documentation in the electronic medical record
- Collaborate with RNs, CNAs, and the interdisciplinary team
- Follow infection control, safety, and facility protocols
Requirements:
- Active New York State LPN license
- Experience in Long-Term Care, Skilled Nursing, or Rehabilitation preferred
- BLS certification
- Strong communication and teamwork skills
- Compassionate and patient-centered approach
Pay: $1,800.00 per week
