We are seeking an organized, compassionate, and detail-oriented Patient Care Coordinator to join our growing team. This role is essential in ensuring clients receive timely, coordinated, and exceptional care while serving as a primary liaison between patients, caregivers, healthcare providers, and administrative staff.
Position Summary
The Patient Care Coordinator is responsible for coordinating home healthcare services from intake through ongoing care. This position manages client scheduling, communicates with patients and their families, collaborates with healthcare professionals, maintains accurate records, and helps ensure compliance with company policies and applicable healthcare regulations.
The ideal candidate is an excellent communicator with strong organizational skills, the ability to manage multiple priorities, and a passion for providing outstanding customer service in a healthcare environment.
Key Responsibilities
- Coordinate patient admissions and the onboarding process for new clients.
- Schedule and coordinate home healthcare visits with nurses, therapists, aides, and caregivers.
- Maintain accurate patient records and documentation in accordance with company policies and applicable regulations.
- Serve as a primary point of contact for patients, family members, physicians, referral sources, and staff.
- Respond to patient inquiries and resolve scheduling or service concerns in a professional and timely manner.
- Monitor care schedules and communicate changes to patients and assigned clinicians.
- Verify physician orders, referrals, and required documentation before the start of care.
- Collaborate with clinical staff to support individualized care plans.
- Coordinate communication among interdisciplinary care team members.
- Assist with obtaining authorizations and verifying insurance information when applicable.
- Monitor service delivery to help ensure continuity and quality of care.
- Track patient appointments, follow-up visits, and required documentation.
- Maintain confidentiality of patient information in accordance with HIPAA and company privacy policies.
- Prepare reports, maintain databases, and update patient records accurately.
- Support quality assurance initiatives and participate in performance improvement activities.
- Assist with intake coordination, referral management, and patient registration.
- Identify and escalate patient concerns or clinical issues to the appropriate supervisor.
- Maintain professional relationships with referral partners and healthcare providers.
- Participate in staff meetings and ongoing training programs.
- Perform additional administrative and patient support duties as assigned.
Required Qualifications
- High school diploma or equivalent required.
- Associate's or bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a related field preferred.
- Minimum of two years of experience in healthcare coordination, medical office administration, home healthcare, case management support, or a related role.
- Strong understanding of healthcare terminology and medical documentation.
- Excellent organizational and time-management skills.
- Outstanding written and verbal communication abilities.
- Strong customer service and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information with professionalism.
