Nevada Guardian Services is a licensed professional fiduciary firm based in Las Vegas. Since 2007, we have served as court-appointed guardians, trustees, and personal representatives for adults who can no longer manage their own affairs. About the Role
This is not a typical RN role. You will not be administering treatments, starting IVs, managing medications, or providing direct hands-on patient care. Instead, your RN training and years of clinical experience will be applied where they matter most for our work: careful observation, sound clinical judgment, and the kind of trained eye that can spot a subtle decline, recognize an emerging concern, or notice when something has shifted with a client.
You will work in close communication with our Certified Guardians, who manage the full scope of each client's legal, financial, and personal affairs. Your role is to be their clinical partner in the field — the trained set of eyes and ears providing the information they need to make sound decisions on behalf of our clients.
This is a primarily out-of-office role. You will spend the majority of your time visiting clients across the Las Vegas valley — in private homes, assisted living, skilled nursing facilities, group homes, and hospitals.
You will book your own appointments and manage your own schedule. Within reason, you have the autonomy to structure your days as you see fit — as long as the work gets done efficiently and clients are seen on their required cadence. This is a role for someone who can be trusted to run their own calendar without close supervision.
Responsibilities
- Visit clients on a regular, scheduled cadence across the Las Vegas valley
- Book and manage your own appointments around each client's required visit frequency
- Conduct welfare and wellness assessments in homes, assisted living, skilled nursing, group homes, and hospitals
- Document each client's current physical, cognitive, and behavioral state with care and precision
- Communicate observations and clinical findings directly to the assigned Certified Guardian
- Identify clinical changes and flag emerging trends to the office team
- Make day-to-day field decisions within a certain scope
- Escalate complex clinical issues to the office for fiduciary follow-up
- Coordinate as needed with facility staff, physicians, and transport companies
- Maintain documentation in alignment with NGA Standards of Practice
Required Qualifications
- Active Nevada RN license (or current compact-state license valid in Nevada)
- Minimum 5 years of home health nursing experience — this is a firm requirement and applications without it will not be considered
- Willingness to make this your primary professional role — not a supplemental position
- Strong clinical assessment and documentation skills
- Reliable personal vehicle and valid Nevada driver's license
- Ability to pass a background check
- No personal bankruptcy on record (required for our fiduciary oversight environment)
- Comfort working independently in the field
Preferred Qualifications
- Experience with geriatric, hospice, or palliative populations
- Familiarity with cognitive assessments (MoCA, Mini-Cog, etc.)
- Prior experience working with adults under guardianship or conservatorship
- Comfort with case management software and digital documentation tools
