Assess clients’ needs and develop individualized service plans. Conduct comprehensive intakes, screenings, and ongoing assessments to identify barriers, strengths, and goals. Secure necessary referrals and coordinate services with internal and external providers to ensure continuity of care. Advocate on behalf of clients to access resources, benefits, housing, healthcare, and employment services.
Maintain accurate, timely, and confidential documentation of all client activities, services, and case notes in compliance with program, funder, and regulatory requirements. Meet regularly with clients to provide ongoing guidance, crisis intervention, and support to help them achieve and maintain self-sufficiency and self-determination. Monitor client progress and adjust service plans as needed.
Facilitate scheduled daily activities and group sessions. Inspect program areas to ensure compliance with program policies, safety standards, and procedures. Assist and support clients in developing problem-solving, communication, and decision-making skills necessary for successful independent living.
Ensure program policies and rules are followed and maintain proper documentation of all activities, incidents, logs, and write-ups. Complete daily intakes, logs, and reports as required. Obtain and maintain HMIS and Skilled Assessor certifications. Process all clients into HMIS in a timely manner and report space availability information to DFSS as needed.
Collaborate with multidisciplinary teams, supervisors, and community partners to coordinate care and participate in case conferences, staff meetings, and trainings. Maintain impeccable records for monthly, quarterly, and annual reports and audits. Maintain professional boundaries and ethical conduct with clients, coworkers, and staff.
Ability to navigate stairs frequently and to walk or stand for prolonged periods. Ability to implement and follow the Director’s directives is required.
