POSITION OVERVIEW
As a PRN Physical Therapist (PT), you will provide skilled home health physical therapy services to adult and geriatric patients in their homes.
With strong patient demand throughout Galveston, you'll have consistent opportunities to pick up visits that fit your schedule.
KEY RESPONSIBILITIES
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs.
- Provide skilled physical therapy interventions in patients' homes.
- Improve patients' mobility, strength, balance, and functional independence.
- Monitor patient progress and modify treatment plans as appropriate.
- Complete accurate documentation using WellSky/Kinnser EMR.
- Educate patients and caregivers on home exercise programs, fall prevention, and safety techniques.
- Collaborate with physicians, nurses, and interdisciplinary healthcare team members.
REQUIREMENTS
- Active Texas Physical Therapist (PT) License
- Current CPR Certification
- Home Health experience preferred
- WellSky/Kinnser experience is a plus
- Reliable transportation
- Valid driver's license and current auto insurance
