Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.
- Monitor and document patient health status, including vital signs, medication adherence, and behavioral changes.
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation.
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary.
- Maintain a clean and safe environment through regular cleaning, laundry, and organization tasks.
- Support clients with mobility using equipment such as Hoyer lifts or wheelchairs, ensuring safe transfer procedures.
- Follow care plans meticulously, including medication administration and patient monitoring, while adhering to HIPAA regulations for confidentiality.
- Collaborate with healthcare professionals to update care strategies and ensure comprehensive support for individuals with complex needs such as dementia, Alzheimer's, or developmental disabilities.
- Engage in patient observation to identify changes requiring medical attention or intervention.
- Assist with community outings or appointments to foster social engagement and independence.
Experience
- Prior experience in home health care, assisted living, nursing homes, or group homes is highly desirable.
- Demonstrated knowledge of first aid, CPR certification, and emergency response procedures.
- Experience working with individuals with disabilities, autism spectrum disorder, dementia, or memory care needs.
- Familiarity with care plans, medication administration, vital signs monitoring, and HIPAA compliance.
- Hands-on experience in patient observation and reporting behavioral or health changes accurately.
- Ability to perform heavy lifting safely when assisting clients with mobility or transfers using equipment like Hoyer lifts.
- Background in long-term care settings or hospice care is a plus for understanding complex patient needs.
