Position Summary
The Licensed Vocational Nurse (LVN) provides skilled nursing care under the direction of a Registered Nurse (RN) and in accordance with physician orders, agency policies, and state and federal regulations. The LVN is responsible for delivering safe, effective, and compassionate patient care while promoting independence and improving overall quality of life.
This position requires strong clinical judgment, excellent communication skills, and the ability to manage patient care in a home setting.
Essential Job Responsibilities
- Provide skilled nursing care in patients' homes according to the physician's plan of care.
- Monitor and document patient conditions, vital signs, symptoms, and progress.
- Administer medications and treatments as ordered by the physician.
- Perform wound care, dressing changes, catheter care, ostomy care, and other skilled nursing procedures.
- Educate patients and family members on disease management, medications, safety precautions, and home care techniques.
- Observe patients for changes in condition and promptly communicate concerns to the supervising Registered Nurse.
- Maintain accurate, timely, and complete clinical documentation.
- Collaborate with physicians, therapists, case managers, social workers, and other members of the healthcare team.
- Ensure compliance with agency policies, HIPAA regulations, infection control standards, and state and federal guidelines.
- Participate in patient care conferences and interdisciplinary team meetings as needed.
- Promote patient independence while ensuring a safe home environment.
- Maintain professionalism and compassionate care during every patient interaction.
Qualifications
- Current California Licensed Vocational Nurse (LVN) license in good standing.
- Current CPR/BLS certification.
- Valid California driver's license.
- Reliable transportation with current automobile insurance.
- Ability to travel throughout Riverside County.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently while managing multiple patient visits.
- Strong documentation and time-management skills.
- Home health experience preferred but not required.
- Electronic Medical Record (EMR) experience is a plus.
Preferred Qualifications
- Previous experience in home health, hospice, skilled nursing, or acute care.
- Knowledge of Medicare home health regulations.
- Experience with wound care and chronic disease management.
- Bilingual candidates are encouraged to apply.

SHIFTCARE - $30/HOUR
PER DIEM VISIT $45/VISIT
