Join our team and make a meaningful impact every day. We’re looking for a compassionate Registered Nurse in the Homeheath field in Milwaukee County. In this role, you'll build strong patient relationships, collaborate with a supportive team, and help families navigate an important life journey.
RN Hospice Key Responsibilities:
- Perform initial home health nursing evaluations within 24 hours of hospice referral.
- Assess physical, emotional, and environmental needs of patients and families every 15 days or as needed.
- Develop, implement, and revise individualized care plans based on patient needs.
- Collaborate with the interdisciplinary team, attending physicians, and families to deliver comprehensive care.
- Document patient progress, goals, and interventions accurately and timely.
What You’ll Need:
- RN license (Wisconsin) + 1 years of home health or hospice experience
- Valid driver’s license and reliable transportation
