Job Coach – Per Diem

Salary: $20 - $22/hr

In this role, you will:
- Support the development and ongoing revisions to individual Staff Action Service Plan by providing input and information.
- Track individual progress towards desired goals and objectives in Life Plan.
- Segment job duties and assignments into specific steps to facilitate the learning of the people we serve.
- Monitor individual level of performance including determining when support can be maintained, increased, or gradually faded.
- Represent agency in a professional and exemplary manner, while in assigned community worksites. Serve as a liaison between people we serve and worksite employees/employers to facilitate appropriate work readiness skills training and professional work relationships.
- Maintain accurate records on job coaching services provided to each person we serve.
- Respond to crisis situations in a humane and proactive manner and in accordance to policy procedures and training.
- Develop and maintain written documentation, as requested.
We require:
- High School Diploma or equivalent
- Qualified & Valid NYS Driver’s License
- At least six (6) months employment experience
- Good math and communication skills
