As a Home Care Aide, you will work closely with clients and their families to develop individualized care plans that address their unique needs and preferences, fostering a supportive and nurturing environment.

- Personal Care: Assist clients with activities of daily living, including bathing, grooming, dressing, and toileting, while respecting their dignity and privacy.
- Mobility Assistance: Help clients with mobility issues to move safely around their homes, using assistive devices as needed, and implementing fall prevention strategies.
- Medication Management: Support clients in adhering to their medication schedules, including reminders to take them, while ensuring compliance with prescribed dosage and timing.
- Nutritional Support: Prepare nutritious meals and snacks according to dietary guidelines and individual preferences, promoting health and well-being through proper nutrition.
- Emotional Support: Provide companionship and emotional support to clients, engaging in meaningful conversations, listening attentively to their concerns, and offering encouragement and empathy.
- Social Engagement: Facilitate social activities and outings that promote socialization and community integration, recognizing the importance of maintaining connections and relationships for overall well-being.
- Health Monitoring: Collect and record clients' vital signs, symptoms, and overall health status.

- Documentation: Maintain accurate and timely documentation of client care activities, observations, and progress notes, adhering to regulatory requirements and agency policies.

- High school diploma or equivalent; certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) required
- Previous experience in home care, nursing, or caregiving necessary
- Fluency in English is required to ensure clear communication with clients and the care team. Bilingual (Spanish) skills are a plus.
- Reliable transportation with proof of insurance, with the ability to drive client's to doctors appointments and/or run errands as needed.
- Strong interpersonal and communication skills, with the ability to establish rapport and build trusting relationships with clients and their families.
- Empathy, compassion, and a genuine desire to help clients achieve health, wellness, and longevity.
- Knowledge of basic medical terminology, safety practices, and infection control procedures.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Must pass ACHA background checks and health screenings as required by regulatory agencies.
