Youth Services Coordinator/Case Manager (Supervisor) -

This full time position is responsible for the coordination and oversight of mental health rehab services both onsite and offsite for youth clients in Dorchester County. Essential job functions include supervision of multiple case manager/rehabilitation staff, scheduling and monitoring the quality of programming and documentation. In addition, this position provides effective leadership to the clients and staff in collaboration with other professional agencies and management staff. The Coordinator will support clients and their families in admissions and intakes to services as well as transitioning and discharge planning into other services. Strong supervisory skills, communication, written, oral and team work skills required.
Bachelor’s Degree Required
Essential job functions and requirements include:
- Supervisory experience highly preferred
- Team oriented, flexible, energetic, and has a commitment to working with people recovering from a severe mental illness
- Delivering services to include supervision of client welfare, individual meetings, medication monitoring, meal preparation, etc.
- Completion of required documentation in our electronic health record
- Linking clients to resources in the community
- Work collaboratively and professionally within a team setting, with other agencies, and client support network
- Provides transportation in company vehicles to clients for service delivery
- Maintains valid driver's license, a clear driving record, and has evidence of auto liability insurance
- Background check required and maintains a clear criminal background profile, drug testing required. Channel Marker is a drug free workplace.
