Shift - Day shift, Monday – Friday, 40-hours per week

In this vital role, you will utilize your extensive nursing background to assess patient needs, develop individualized care plans, and facilitate seamless transitions across various healthcare settings.
Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to determine appropriate care strategies.
- Coordinate inpatient and outpatient services, ensuring smooth transitions from hospital to post-acute or community-based care settings.
- Develop personalized discharge plans that incorporate medical records review, diagnostic evaluations, and patient education to optimize recovery outcomes.
- Manage utilization review processes by evaluating medical necessity and supporting appropriate resource allocation within managed care frameworks.
- Collaborate with multidisciplinary teams to deliver holistic patient care.
- Oversee intake procedures for new patients, ensuring accurate documentation and compliance with HIPAA regulations for patient confidentiality.
- Perform ongoing patient service activities like case documentation in medical records systems and conducting diagnostic evaluations to inform treatment pathways.
Qualifications
- At least 2 years of case management experience.
- 6-months discharge planning Case Management experience
- Valid Massachusetts RN license
