What You'll Do
- Perform comprehensive wound and skin assessments.
- Develop, implement, and monitor individualized wound care treatment plans.
- Complete routine wound rounds and accurate clinical documentation.
- Collaborate with physicians, therapy, dietary, nursing, and interdisciplinary teams to optimize healing outcomes.
- Monitor residents for changes in skin integrity and implement preventative interventions.
- Educate nursing staff, residents, and families on wound prevention and treatment.
- Ensure compliance with state, federal, and facility policies regarding wound care and infection prevention.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to skin integrity and wound management.
- Serve as a clinical resource for pressure injury prevention and best practices.
Qualifications
- Current Illinois RN or LPN license in good standing.
- Previous long-term care or skilled nursing experience preferred.
- Wound care experience strongly preferred.
- Wound Care Certification (WCC, WOCN, or similar) is preferred but not required.
- Strong assessment, documentation, and communication skills.
- Passion for providing compassionate, resident-centered care.
