Job Description:
A Certified Community Behavioral Health Clinic (CCBHC) provider in Queens seeks qualified and motivated individuals to work with a multidisciplinary team of behavioral health, substance use, primary care, psych rehab services specialists and peer support staff to provide integrated services to children and adults with behavioral health and/or substance use issues.
CCBHC Targeted Case Management (TCM) services are intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. CCBHC Targeted Case Management services are organized around goals aimed at providing access to services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence or self sufficiency and maintain themselves in the community.
Job Responsibilities:
Promote hope and recovery by using strengths-based, culturally appropriate, and person-centered practices; Maximize community integration and normalization; Provide leadership in ensuring the coordination of resources for individuals eligible for behavioral health services; To achieve wellness and recovery goals, this service may include the following components: * Service Referral, Linkage, and Related Activities
Service components shall support the individual obtain and maintain needed services and may include:
o Referral to community and social support services
o Providing support for improved community service linkages
o Transitional care, including appropriate follow-up from inpatient to other settings
o Support in navigating complex health care and social services systems
o Individual and family support
o Advocacy for needed services
o If the recipient is eligible, working to secure Medicaid benefits with the goal of subsequent Health Home enrollment
o Performing on-site crisis intervention and skills teaching when other services are not available
* Monitoring and Follow-up:
Monitoring and follow-up as needed with individual, family, service providers and other
entities to ensure that required and desired services are consistent with the individual’s needs and goals
.
This is a full time position which requires two weekdays from 11AM to 7 PM and three days from 9 to 5, one being on the weekend.Two consecutive days off will be provided if preferred.
Skills, experience and assets:
Bachelor Degree (minimal) in psychology, human services or related fields.
Experience in providing Care Coordination, Care Management and/or Case Management services inside and outside of the facility to assist individuals in identifying and linking to community based services, health and wellness providers and benefits necessary to promote the health, wellness, recovery, stability, autonomy and life setting outcomes desired by the individual.
Experience working in or with behavioral health, substance use and/or other social services setting.
Knowledge of computers and office applications for electronic medical records documentation.
Ability and willingness to travel in the served community to assist individuals when required.
Willingness to work collaboratively with others.
Flexibility in participating in this newly developed integrated care setting.
Fluency in Spanish and other languages is an asset.
