Overview
We are seeking a highly skilled and compassionate Nurse Practitioner to join our multidisciplinary healthcare team. Job Title: Nurse Practitioner
Location: Richland County, SC (2 facilities)
Duration: 6+ month contract to hire
Shift: Mon-Fri 8-5pm
Pay Rate: 65-70/hr
Specific Duties:
- Provide specialized care to high-risk health plan members
- Work as part of an interdisciplinary team
- Conduct comprehensive assessments
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate documentation, including ICD-10 coding
- Communicate and collaborate with the care team
- Facilitate advanced illness and care planning discussions
- Offer counseling and education to patients and caregivers

Must have qualifications/experience:
- Nurse Practitioner or Physician Assistant
- NP Certified in one of the following (FNP, ANP, GNP)
- Access to transportation
- 1+ year of experience (LTC, SNF, Geriatric preferred)
