Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; document findings accurately using Electronic Health Record (EHR) systems
- Support medication administration following prescribed schedules and medical terminology guidelines
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments; deliver memory care and hospice support as needed
- Perform catheter care and manage other medical procedures with precision and adherence to home healthcare regulations
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients
- Utilize assistive devices like Hoyer lifts or transfer belts to ensure safe patient mobility and prevent injury during transfers
Requirements
- Proven experience in assisted living facilities, nursing homes, or home healthcare settings; familiarity with geriatric care and working with individuals with disabilities or developmental disabilities
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring
- Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS); first aid training is a plus
- Ability to lift heavy objects safely and perform physically demanding tasks required for patient care
- Experience working with EMR/EHR systems for accurate medical documentation and records management
- Strong understanding of home & community care regulations and HIPAA compliance to ensure patient privacy and safety
- Excellent caregiving skills combined with compassion, patience, and a proactive attitude toward patient well-being
