Job Summary
The behavioral health care manager will be a core member of the collaborative care team, including the patient’s primary care provider and psychiatric nurse practitioner consultant, as well as the larger primary care team. The behavioral health care manager will be responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, the psychiatric nurse practitioner provider.
Duties
- ▪ The ability to provide brief psycho-social interventions such as problem-solving treatment.
- ▪ Demonstrated ability to collaborate and communicate effectively in a team setting.
- ▪ Ability to maintain effective and professional relationships with patient and other members of the care team.
- ▪ Experience with screening for common mental health and/or substance abuse disorders.
- ▪ Ability to effectively engage patients in a therapeutic relationship, when appropriate.
- ▪ Ability to work with patients by telephone as well as in person.
- ▪ Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.
Qualifications
- Preferred to have a minimum of 2 years post-graduate experience providing behavioral healthcare support services to patients, preferably in the community, medical office or school setting; willing to train.

This is a new program and position that will start with part-time hours and move to full-time as it develops. Salary is dependent upon education, licensure, and experience. Health insurance, ALFAC, PTO. Paid holidays. Dedicated time during working hours for case team meetings with psychiatric nurse practitioner and providers.
