About the Role
Smart Rehab is growing and seeking Occupational Therapists to provide Medicare Part B outpatient therapy in patients' homes in the St. Charles area. This is a 1099 role with a flexible, self-directed schedule and a caseload that grows as our referral base expands.
This is outpatient (Part B) care delivered in the home, not traditional home health. You work one-on-one with motivated older adults in 60-minute sessions, directing your own plan of care without productivity quotas or agency oversight. It's well suited to OTs looking for flexible, supplemental, or part-time visits, with room to take on more as our caseload builds.
Responsibilities
- Complete OT evaluations, treatment sessions, and discharges in the patient's home
- Develop and direct your own plans of care for an older-adult outpatient population
- Provide one-on-one, 60-minute therapy sessions focused on mobility, strengthening, balance, fall prevention, and functional independence
- Document visits using the provided EMR platform (training included)
- Provide caregiver training and home safety recommendations
- Coordinate with referring providers as needed
Schedule & Workload
- Flexible, self-directed scheduling — you choose when you work
- Part-time and PRN availability, with volume growing as referrals increase
- No weekends or evenings required
- No productivity quotas
Requirements
- Missouri-licensed Occupational Therapist
- Experience with older adults preferred
- Medicare Part B provider, or willingness to enroll
- Strong communication and documentation skills
- Comfortable working independently in home environments
Compensation & Structure
- $65–$75 per visit (1099, independent contractor)
