Job Summary
We are seeking a compassionate and dedicated Personal Care Assistant/Certified Nursing Assistant to join our team and provide exceptional support to individuals in need of assistance with daily activities and healthcare needs. This vital role involves delivering personalized care in various settings, including home healthcare, senior living communities, and personal care homes. Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor and observe patients’ conditions regularly, documenting vital signs, medical records, and changes in health status accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems such as Alora Plus.
- Assisting with self-administration of medications following home healthcare regulations and ensuring proper documentation while maintaining patient confidentiality in compliance with HIPAA standards.
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care techniques and behavioral management strategies.
- Support clients with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment conducive to health and well-being.
- Communicate effectively with clients, families, and healthcare teams to coordinate care plans and ensure client comfort and safety during home health or hospice services.
Skills
- Strong knowledge of medical terminology, patient care procedures, and home health care regulations.
- Experience working with geriatric patients, individuals with disabilities, or those requiring hospice or dementia care.
- Proficiency in using EMR/EHR systems such as Alora Plus for documentation of patient information and medical records.
- Excellent patient observation skills to identify changes in condition or behavior promptly.
- Ability to perform CPR, First Aid, Basic Life Support (BLS), and other emergency response techniques confidently.
- Familiarity with patient transfer methods including Hoyer lifts and safe handling practices for heavy lifting.
- Exceptional communication skills for engaging effectively with clients’ families and multidisciplinary healthcare teams while maintaining patient confidentiality under HIPAA guidelines.

Pay: $17.00 - $18.50 per hour
