Job Summary
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our home health care team in Aurora, CO. In this role, you will provide direct care and comprehensive case management for a designated team of clients in accordance with the state Nurse Practice Act and agency policies. You will also lead, supervise, and mentor a talented care team consisting of RNs, LPNs, Home Health Aides, and Personal Care Assistants to ensure the highest standards of clinical excellence.
Schedule & Work Structure (Hybrid)
This is a full-time, structured hybrid position:
- 2 Remote Days: Working from home.
- 1 Office Day: In the Aurora office on Wednesdays.
- 2 Field Days: Conduct in-home supervisory visits and assessments with clients.
Compensation
- Salary Range: $42.00 - $45.00 per hour (Salary position, rate based on experience).
What You Will Do (Essential Functions)
- Client Assessments & Care Planning: Perform initial and ongoing comprehensive assessments (including OASIS data collection) to determine client eligibility and develop coordinated, high-quality care plans in collaboration with physicians and multidisciplinary teams.
- Clinical Supervision: Manage, mentor, and evaluate an assigned team of RNs, LPNs, HHAs, and PCAs. Conduct in-home supervisions, identify educational needs, and lead regular team meetings.
- Quality & Compliance: Monitor clinical documentation to ensure absolute compliance with regulatory standards of timeliness, accuracy, and completeness. Ensure care aligns with state, federal, CHAP, and Joint Commission regulations.
- Collaboration & Advocacy: Act as a critical link between clients, families, and physicians—identifying subtle changes in client conditions and advocating for immediate and long-range care needs.
- Safety & Confidentiality: Maintain strict client confidentiality and promote a safe environment through strict adherence to infection control and safety protocols.
Qualifications & Requirements
- Education: Graduate of an accredited school of professional nursing (Baccalaureate degree preferred).
- Licensure: Current, unrestricted license as a Registered Nurse (RN) in the state of Colorado.
- Experience: Minimum of two (2) years of nursing experience. Home care experience is highly preferred.
- Skills: Strong clinical assessment skills (OASIS experience preferred), excellent organizational/time-management habits, and top-tier written and verbal communication skills.
- Transportation: Valid driver’s license with an insured automobile in good repair (required for field days)
Pay: $42.00 - $45.00 per hour
