Job Summary

Responsibilities
- Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor and observe patients’ conditions regularly, noting changes in health status and reporting to supervising healthcare providers.
- Assist with medication administration following prescribed protocols and maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Support patients with medical procedures such as catheter care, vital signs measurement, and basic wound care while adhering to home healthcare regulations.
- Facilitate patient transfers using appropriate methods like Hoyer lifts or patient transfer techniques to ensure safety and comfort.
- Offer companionship and emotional support, especially for clients with dementia, Alzheimer’s disease, or those receiving hospice or end-of-life care.
- Maintain patient confidentiality in compliance with HIPAA standards and ensure a clean, safe environment through light housekeeping tasks such as cleaning and laundry.
- Communicate effectively with clients and their families about care plans, medication schedules, and health updates.
- Follow all safety protocols including first aid, CPR certification requirements, and emergency response procedures to ensure client safety at all times.
Experience
- Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred.
- Experience working with individuals with developmental disabilities, geriatrics, dementia care, or hospice is advantageous.
- Familiarity with home health care assistance tools such as Hoyer lifts and patient transfer methods is desirable.
- Knowledge of medical terminology, medication administration procedures, and vital signs monitoring is essential.
- Proven ability to work within home health agency settings while maintaining compliance with home health care regulations.
- Previous experience using EMR/EHR systems for documentation is a plus.
- Strong communication skills combined with compassionate caregiving approach are necessary to succeed in this role.

Pay: $33.18 - $37.91 per hour
