Schedule/Hours:
- 13-week full-time assignment
- Monday through Friday
- Daily visits scheduled; on-call likely as needed
Key Responsibilities:
- Deliver high-quality nursing care in patients’ homes, utilizing advanced clinical skills
- Complete thorough patient assessments and OASIS documentation promptly and accurately
- Work closely with physicians and interdisciplinary teams to create and implement individualized care plans
- Perform wound care, medication management, disease monitoring, and patient/caregiver education
- Communicate patient condition updates efficiently with the clinical team to ensure coordinated care
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a Home Health RN
- Required experience with OASIS documentation
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid driver’s license
- Ability to manage the demands of a full-time, structured caseload with professionalism
