Job Description:
A Place 4 Me in Concord, NC is seeking a compassionate and dedicated Community Support Team Therapist to join our team. As a Community Support Team Therapist, you will provide clinical and administrative supervision to the team while also functioning as a practicing clinician. Your role will involve planning, coordinating, and overseeing therapeutic and rehabilitative services for individuals enrolled in Community Support Team Services. Responsibilities:
- Provide clinical oversight, coordination, and supervision of the Community Support Team (CST) members, including QPs, Associate Professionals, and Paraprofessionals.
- Develop and maintain comprehensive Person-Centered Plans (PCP) for clients, incorporating ongoing assessments, interventions, and revisions as needed.
- Coordinate and facilitate person-centered planning, revisions, and updates through regularly scheduled treatment team meetings, involving formal and informal supports as appropriate.
- Establish cooperative and collaborative working relationships with members of the client system, acting as an FPS (A Place 4 Me) Ambassador and positively representing the organization.
- Ensure that interventions are based on assessment recommendations, supported by documented medical necessity, and implemented as documented in the PCP.
- Measure client outcomes and evaluate the effectiveness of interventions, providing ongoing evaluation and adjustments to the PCP.
- Mentor and provide case consultation/guidance to Para-Professionals and Associate Professionals to support the implementation of interventions as prescribed by the PCP.
- Make referrals to recommended services and ensure continuity of care for clients transitioning across different levels of care.
- Develop crisis stabilization plans for clients and perform "first responder" duties of crisis response as assigned.
- Provide case management functions, including linking, referring, arranging, and coordinating services for clients.
- Address barriers that impede the development of skills necessary for independent functioning in the community.
- Complete required client-related reports, attend court as requested, and provide information for court reports as required.
- Coordinate discharge planning for clients in hospitals, residential services, or other levels of care, as well as assist with community re-entry following hospitalization or other levels of care.
- Adhere to program requirements, clinical coverage policies, and regulatory requirements for utilization of Community Support Team Services.
- Maintain client confidentiality in adherence to HIPAA regulations.
- Ensure accurate and timely clinical documentation of all services, interventions, and client-related activities.
- Actively participate in staff and supervisory meetings, in-service trainings, and staff development activities.
- Identify and communicate training and supervision needs to ensure services are provided within the scope of expertise and abilities.
- Maintain established productivity requirements for delivering direct, billable services to assigned clients.
Qualifications:
- Master's degree in Social Work, Psychology, or a related human services field from an accredited college or university.
- Provisional or full licensure as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), or Licensed Marriage and Family Therapist (LMFT) within the state of North Carolina.
- Minimum of 1 year of experience providing direct service to the population served in mental health, developmental disabilities, or substance abuse settings.
- Credentialing as a Qualified Professional (QP) according to the requirements specified in 10A NCAC 27.G.0104.
- Knowledge of mental health or human services systems, adult psychopathology, person-centered planning philosophy, evidenced-based best practices, family systems, and family dynamics.
- Familiarity with medical/clinical and Medicaid documentation standards and practices, community resources, cultural competence, DSM-IV, and mandated reporting requirements.
- Strong time management, interpersonal communication, and documentation skills.
- Ability to establish and maintain effective clinical relationships, boundaries, and professional working relationships with clients, families, collaterals, and other professionals.
- Proficiency in using a computer and software applications such as email and Word.
Other Details:
- This position may require travel approximately 50% of the time.
- Evening and weekend availability is required for all CST team members.
