Job Summary:
Central City Health is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The Registered Nurse (RN) provides care to patients/families by assessing, triaging, planning, and intervening on actual or potential health problems, including evaluating patient responses to care. These responsibilities also include patient/family education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals. They must demonstrate knowledge of the principles of human growth and development, as well as chronic disease management, and interpret data reflective of the patient's status to identify requirements relative to his or her specific needs.
Responsibilities:
- Ensures all actions represent the organization in a highly professional manner.
- Ensures timely, ongoing communication with the provider staff and care team(s).
- Ensures effective and efficient comprehensive transitional care management.
- Communicate with patients, patients’ family, PCP, and care team post hospitalization.
- Maximizes the effectiveness of patient office visits.
- Coordinates reviews of patients’ medication effectiveness with the medical team.
- Reviews lab results daily; discusses abnormal results with ordering provider.
- Assist patients in setting/maintaining self-management objectives.
- Recommends interventions to modify behaviors associated with health risk.
- Assesses, plans, and provides nursing care for patients.
Education & Experience:
- Associate degree in nursing (ADN) required; bachelor’s degree preferred
- Valid State of Michigan Registered Nurse licensure required.
- BLS certification required; ACLS certification preferred.
- Minimum of two (2) years of recent nursing experience in a community-based setting
- Must meet all Continuing Education requirements.
- Must be able to work cooperatively with staff
- Experience in managing multiple tasks simultaneously and meeting critical deadlines.
- Must maintain confidentiality relating to treatment in accordance with guidelines.
- Knowledge of PCMH preferred.
