Community Prenatal Care Coordinator (PNCC) Grow With Us Prenatal Care Agency Milwaukee, WI Community-Based | Flexible Schedule Job Overview Grow With Us Prenatal Care Agency is seeking compassionate, motivated, and self-driven individuals to join our growing team as Community Prenatal Care Coordinators (PNCC). Please read before applying. This is a community outreach position and is not a traditional office job. Care Coordinators are responsible for finding, recruiting, and enrolling their own eligible prenatal clients through networking, community outreach, referrals, and relationship-building. This position does not include an assigned caseload. Applicants must be comfortable building and maintaining their own client caseload through community outreach and networking. Responsibilities Recruit and enroll eligible pregnant women into the PNCC program. Build relationships with healthcare providers, OB/GYN offices, hospitals, WIC clinics, community organizations, schools, shelters, and referral partners. Conduct community outreach, networking, and educational activities. Complete eligibility screenings, assessments, care plans, and required documentation. Connect clients with community resources including: Housing Food assistance Transportation Parenting education Breastfeeding support Baby supplies Mental health resources Other supportive services Maintain accurate and timely documentation. Represent Grow With Us Prenatal Care Agency professionally throughout the community. Qualifications Excellent communication and interpersonal skills. Self-motivated and able to work independently. Comfortable conducting community outreach and networking. Passion for helping pregnant women and families. Reliable transportation required. Experience in healthcare, case management, social work, human services, education, customer service, or community outreach is preferred but not required. Willingness to complete all required PNCC training. Must successfully pass all required caregiver background checks. Compensation Compensation will be discussed during the interview process. The hourly wage displayed on Indeed is an estimated range required for posting purposes. Final compensation will be based on the agency's compensation structure and successful completion of all onboarding requirements.
