Integrated Counselor (LMSW / LMHC / LCSW)

CASA-Trinity is seeking an Integrated Counselor to provide mental health and substance use disorder services within our integrated care model. This position promotes whole-person treatment through collaboration with medical, psychiatric, substance use, and peer recovery providers to address clients’ medical, psychiatric, and psychosocial needs.
The Integrated Counselor delivers diagnostic assessment, psychotherapy, crisis intervention, and care coordination as part of a multidisciplinary team. Responsibilities and level of autonomy vary based on licensure.
Essential Job Functions (All Licensure Levels)
- Conduct diagnostic evaluations and comprehensive psychosocial assessments for individuals with mental health and co-occurring substance use disorders.
- Provide individual, family, and group counseling using evidence-based and trauma-informed approaches.
- Develop, implement, and update individualized, integrated treatment plans with measurable goals.
- Provide crisis assessment and intervention, including safety planning and risk management.
- Collaborate with substance use, psychiatric, medical, and peer recovery staff to ensure coordinated, integrated service delivery.
- Document services in the electronic health record in a timely and accurate manner, in compliance with NYS OASAS, OMH, Public Health Law, 42 CFR, HIPAA, and agency policy.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement activities.
- Maintain professional competence through continuing education and adherence to best practices.
- Perform other related duties as assigned.
Role Differentiation by LicensureLMSW
- Provides counseling and assessment services under clinical supervision of an independently licensed LMHC or LCSW.
- Participates in regular supervision and professional development toward independent licensure.
- Supports care coordination, case management, and referrals to community resources.
- Functions within NYS LMSW scope of practice and licensure regulations.
LMHC / LCSW (Independent License)
- Functions autonomously within professional scope, providing advanced clinical and diagnostic services.
- Provides leadership within the integrated care model and serves as a clinical resource to the treatment team.
- Offers clinical consultation and mentorship to LMSWs and CASAC staff.
- Contributes to clinical policy development, staff training, and program improvement initiatives.
- Participates in quality assurance and integrated practice leadership activities as directed.
Knowledge, Skills, and Abilities
- Knowledge of mental health, substance use, and co-occurring disorder treatment.
- Understanding of integrated behavioral health and collaborative care models.
- Strong assessment, treatment planning, documentation, and clinical reasoning skills.
- Ability to deliver culturally competent, recovery-oriented, trauma-informed care.
- Excellent communication, organizational, and interpersonal skills.
- Proficiency with electronic health record systems.
- Leadership, mentorship, and supervision skills (LMHC/LCSW).
Physical Demands
- Use of computer and standard office equipment.
- May occasionally lift or move up to 25 pounds.
- Ability to manage crisis situations and high-stress clinical environments.
- Possible exposure to illness; must adhere to safety protocols.
Work Environment

- Work performed primarily in a clinical or office setting.
Qualifications
Required (All Levels):
- Master’s degree in Social Work, Counseling, or related field.
- Valid New York State licensure (LMSW, LMHC, or LCSW).
- Valid driver’s license.
- Position subject to criminal background clearance.
Additional Requirements by License:
- LMSW: Must work under supervision of an independently licensed clinician; experience in mental health, substance use, or integrated care preferred.
- LMHC/LCSW: Independent NYS licensure required; minimum of three years post-master’s clinical experience in mental health, substance use, or co-occurring treatment settings preferred.
