We are seeking a dedicated and organized Experienced Full time and Part Time Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care by coordinating their services and facilitating communication between patients, healthcare providers, and administrative staff. This position is essential in enhancing patient experiences and improving health outcomes.
Duties
- Serve as the primary point of contact for patients, addressing inquiries and providing information regarding their care.
- Coordinate appointments, referrals, and follow-up care to ensure seamless patient experiences.
- Maintain accurate patient records and documentation in compliance with medical regulations.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Assist in managing office operations within a medical office setting, ensuring efficient workflow.
- Utilize medical terminology to communicate effectively with patients and healthcare professionals.
- Support administrative tasks such as scheduling, filing, and maintaining inventory of office supplies and other tasks as needed.

Requirements
- Previous experience in a office is required.
- Strong understanding of medical terminology .
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Proficient in Microsoft Office Suite and other relevant software applications.
- Strong communication skills, both verbal and written, to interact effectively with patients and team members.
- Ability to work independently as well as part of a collaborative team environment.
- A compassionate approach towards patient care with a commitment to confidentiality and ethical standards.
- CNA is mandatory
