Duties include:
- Communicate effectively with PT regarding the patient, care plan, treatment, and goals.
- Communicate with other members of patient care team and adjust care plan as needed.
- Reach out for community services to assist patient as needed
- Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care.
- Documents direct patient services, coordination and collaboration with physician and other disciplines or provides care for the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy.
Job Requirement:
- Physical Therapist Assistant license to work in CA
- Minimum 1 year of Home Health experience
- CPR / ALS Card
- Driver’s License or State ID Card
- Current Negative TB Test
- Current Physical Exam
- COVID Vaccine Card or Exemption Proof
- COVID Booster Proof
- Able to do Home Health Visits in Pleasanton, Fremont, Livermore and nearby cities
