Job Summary
We are seeking a compassionate and dedicated Direct Care Worker to provide essential support and care to individuals with diverse needs. Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature to track health status
- Administer medications accurately following prescribed schedules and protocols
- Support residents with memory care needs, including Alzheimer’s and dementia care strategies
- Operate equipment such as Hoyer lifts safely to transfer residents with mobility challenges
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
- Maintain cleanliness of resident rooms and common areas to promote hygiene and comfort
- Document care activities using Electronic Medical Record (EMR) systems accurately and promptly
- Engage residents in social activities to promote mental stimulation and emotional well-being
- Collaborate with healthcare professionals, families, and team members to develop personalized care plans
Requirements
- Prior experience in assisted living facilities, nursing homes, or home & community care settings preferred
- Knowledge of ADL assistance, medication administration, vital signs monitoring, and dementia or Alzheimer’s care
- Certifications such as First Aid and CPR are highly desirable
- Experience working with individuals with developmental disabilities or disabilities is a plus
- Ability to operate EMR systems efficiently and accurately record care activities
- Valid driver’s license for driving responsibilities related to resident outings or appointments
- Strong communication skills with a caring attitude focused on resident safety and dignity
- Flexibility to work various shifts including weekends or evenings as needed
