What You'll Do:
As a Remote Case Manager, you will manage a caseload of members with acute, chronic, and/or catastrophic conditions. Your responsibilities will include:
Comprehensive Assessment & Planning

Conduct in-depth telephonic assessments of patients’ clinical, functional, psychosocial, and vocational needs

Review medical records, claims data, and treatment plans to identify risks and opportunities for intervention

Develop individualized, goal-oriented Case Management plans with measurable outcomes
Care Coordination & Clinical Oversight

Coordinate care across providers, facilities, and support services to ensure appropriate, cost-effective treatment

Perform utilization review and apply evidence-based criteria to support level-of-care recommendations

Collaborate with physicians, clients, and interdisciplinary teams on complex or non-standard cases

Identify alternative treatment options and Centers of Excellence when appropriate
Patient Education & Advocacy

Educate patients on diagnoses, treatment options, medications, and lifestyle modifications

Provide ongoing coaching to improve compliance, self-management, and overall health outcomes

Address psychosocial barriers and connect patients to community and support resources
Ongoing Monitoring & Evaluation

Maintain regular contact with patients and care teams to assess progress and adjust care plans

Evaluate treatment effectiveness, patient engagement, and goal achievement

Identify and document outcomes, including cost savings, barriers, and patient satisfaction
Documentation & Communication

Maintain timely, accurate, and compliant documentation of all case activities

Prepare case summaries, client reports, and authorization communications

Track billable time and adhere to regulatory, accreditation, and HIPAA standards

What We’re Looking For:
Education & Experience

Active, unrestricted RN compact license AND one of the following:

Certification as a Case Manager.

Minimum of three years of direct clinical patient care experience.
Skills & Abilities

Exceptional verbal and written communication skills.

Strong problem-solving and decision-making abilities.

Ability to manage multiple cases and prioritize effectively.

Self-motivated and capable of managing responsibilities independently.

Proficiency in Microsoft Outlook and basic computer skills
