Aging Well Rochester, a Division of Reflections Management & Care, is growing and looking to add a compassionate, organized, and dependable Care Manager or Care Manager Assistant to our Rochester care management team.
This is a per diem, part-time position with flexible hours. You are able to make your own schedule based on your availability, client schedules, and client needs. This position is ideal for someone who enjoys working independently, values flexibility, and has a heart for supporting older adults and their families.

Our clients may include:
Older adults living at home, in independent living, assisted living, memory care, skilled nursing, or rehabilitation settings
Individuals living with dementia, Alzheimer’s disease, Parkinson’s disease, chronic illness, mobility changes, or cognitive decline
Families who need guidance, advocacy, communication, and support navigating care options
Long-distance family members who need a trusted professional to be their eyes and ears locally
Individuals transitioning from hospital to rehab, rehab to home, or considering higher levels of care
Caregivers who need education, support, and help coordinating services
Position Overview
The Care Manager or Care Manager Assistant will provide support to clients and families through visits, assessments, care coordination, advocacy, appointment accompaniment, communication, and ongoing monitoring. This role requires strong communication skills, professionalism, sound judgment, and the ability to work independently while remaining connected to the larger care management team.
Responsibilities may vary depending on education, licensure, experience, and client needs.
Job Responsibilities May Include:
Conducting client visits in homes, senior living communities, assisted living facilities, memory care settings, hospitals, rehabilitation centers, or skilled nursing facilities
Assisting with assessments of client needs, safety concerns, care supports, and overall well-being
Developing or supporting care plans based on client goals, family concerns, and identified needs
Accompanying clients to medical appointments and helping communicate updates to family members or providers
Monitoring changes in health, cognition, mood, function, safety, or care needs
Communicating with families, healthcare providers, facility staff, home care agencies, and community resources
Assisting families in understanding care options, services, and next steps
Supporting transitions of care, including hospitalizations, rehab stays, discharges, and moves to higher levels of care
Providing dementia care support, caregiver guidance, and family education
Documenting visits, observations, updates, and recommendations in a timely and professional manner
Helping coordinate services such as home care, medical appointments, transportation, therapy, equipment, and community supports
Advocating for client needs while maintaining dignity, respect, and client-centered care
Identifying concerns and communicating them promptly to the care management supervisor or team
Maintaining professional boundaries, confidentiality, and ethical standards at all times
Education and Experience Needed
Qualified candidates may have a background in one or more of the following areas:
Nursing
Social work
Gerontology
Psychology
Human services
Care management
Home care
Healthcare administration
Aging services
Rehabilitation
Senior living
Dementia care
Preferred education and credentials may include:
RN, LPN, LMSW, MSW, BSW, or related clinical/social service degree
Bachelor’s or master’s degree in social work, nursing, psychology, gerontology, human services, or a related field
Experience working with older adults, individuals with dementia, individuals with chronic illness, or families navigating long-term care
Care management, discharge planning, home care, senior living, skilled nursing, rehab, hospice, or community-based experience preferred
Dementia training, care management certification, Aging Life Care experience, or related certifications are a plus
Care Manager Assistant candidates may be considered with strong experience in senior care, home care coordination, healthcare support, case support, or administrative care coordination, even if they do not hold a clinical license.

This is a per diem, part-time position. Hours vary based on client needs, referrals, appointments, and your availability.
You are able to make your own schedule, while also understanding that client visits, appointments, and family needs may require some flexibility. Most visits occur during daytime hours, though occasional early evening or urgent needs may arise.
