Responsibilities:
- Develop and maintain referral relationships with hospitals, SNFs, ALFs, physicians, and community partners
- Educate healthcare professionals about hospice eligibility and benefits
- Coordinate with clinical staff to ensure smooth admissions
- Manage and update referral pipeline
- Meet and exceed monthly census goals
Qualifications:
- Hospice marketing/liaison experience REQUIRED
- Strong existing relationships in the San Antonio market preferred
- Knowledge of hospice eligibility guidelines
- Self-motivated, driven, and relationship-focused
- Excellent communication and follow-up skills
