Job Title: Nurse Practitioner (NP) – Cardiology & Vascular Services (Nursing Home Rounding)
Position Summary
We are seeking a motivated and clinically skilled Nurse Practitioner (NP) with experience or strong knowledge in Cardiology and Vascular Medicine to provide comprehensive medical care to patients residing in skilled nursing facilities, rehabilitation centers, and long-term care facilities. This position offers a competitive base salary plus productivity-based compensation, allowing providers to increase earnings through patient volume and quality care delivery.
The NP will work collaboratively with physicians, facility staff, and interdisciplinary healthcare teams to evaluate, diagnose, treat, and manage patients with cardiovascular and vascular conditions while promoting continuity of care and reducing avoidable hospitalizations.
Key Responsibilities
- Conduct routine and follow-up patient rounds in skilled nursing and long-term care facilities.
- Evaluate, diagnose, and manage acute and chronic cardiovascular and vascular conditions.
- Perform comprehensive history and physical examinations.
- Develop and implement individualized treatment plans in collaboration with supervising physicians.
- Monitor and manage conditions such as:
- Congestive Heart Failure (CHF)
- Coronary Artery Disease (CAD)
- Atrial Fibrillation and other arrhythmias
- Hypertension
- Peripheral Arterial Disease (PAD)
- Venous Insufficiency
- Deep Vein Thrombosis (DVT)
- Post-operative vascular and cardiac patients
- Order, interpret, and review diagnostic tests, laboratory results, and imaging studies.
- Prescribe medications and adjust treatment plans as clinically indicated.
- Coordinate care with nursing staff, specialists, hospitals, and family members.
- Document patient encounters accurately and timely within the electronic medical record (EMR).
- Participate in quality improvement initiatives aimed at reducing hospital readmissions and improving patient outcomes.
- Educate patients, families, and facility staff regarding cardiovascular and vascular disease management.
Qualifications
- Current, unrestricted Nurse Practitioner license in Wisconsin.
- Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP).
- DEA registration and prescriptive authority.
- Minimum of 1–2 years of Nurse Practitioner experience preferred.
- Previous experience in Cardiology, Vascular Medicine, Internal Medicine, Geriatrics, Skilled Nursing Facilities, or Long-Term Care strongly preferred.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage a mobile rounding schedule.
