Where You'll Work
We serve patients across: McHenry, Kane, Cook, DuPage, Will, Winnebago, and Boone Counties
Field-based role — you'll travel to patients in their homes within your assigned area.
What You'll Do
Registered Nurse - Overview:
- OASIS Start of Care, Recertification and Resumption and case management
- Assessing or re-evaluating the patient's condition and needs to determine homebound status and plan of care
- Provide patient care according to patient's care plan and needs
- Teach the patient and/or family members about care and support skills.
- Medication management and education for patient and/or family.
- Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician.
- Accurately and thoroughly document patient care within company policy.
- Supervision of License Practical Nurse and CNAs.
- Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life.
Qualifications:
- Active Illinois RN or LPN license in good standing
- Valid driver’s license, reliable vehicle, and auto insurance
- Current CPR Certification
- Prior experience in home health, hospice, rehab, or hospital setting is required
- Familiarity with Electronic Medical Records (EMR) software
- Comfortable with technology - a company tablet will be provided
- Strong communication and interpersonal skills
- Time management to balance patient care, travel, and documentation
- Great customer service
