Elite Home Care is a growing, locally operated home health agency serving the Oklahoma City metro. We are hiring a full-time Community Liaison to own referral development across the metro — building and maintaining relationships with physicians, hospital discharge planners, skilled nursing facilities, and assisted living communities so that patients who need home health care find their way to us.
This is a field role. You will spend most of your week out of the office making calls and visits, working a defined territory with warm account lists already built in our CRM. You will be measured on outcomes that matter: referrals that convert to admitted patients, not activity for its own sake.
What you'll do
- Build and maintain referral relationships with physician offices, hospital and SNF discharge planners, ALFs, and other community referral sources across the OKC metro
- Work a defined territory using prepared account lists in our CRM (HubSpot), logging activity and referral signals
- Represent Elite Home Care at facility visits, community events, and provider meetings
- Coordinate hand-offs with our intake team so referred patients start care smoothly and referral sources stay informed
- Participate in a weekly pipeline review with sales leadership
What success looks like
Your single accountability metric is referral-to-admission conversion — referrals you generate that become patients on service. Your bonus is paid on actual commenced starts of care, uncapped: the more patients you help bring on service, the more you earn, with no ceiling.
Compensation and benefits
- Base salary: $50,000–$60,000 depending on experience
- Uncapped performance bonus paid on commenced starts of care

What we're looking for
- Experience in healthcare sales, marketing, or community liaison work — home health or hospice strongly preferred
- Existing referral relationships in the OKC metro (physicians, discharge planners, SNFs, ALFs) are a significant plus
- Valid driver's license, reliable insured vehicle, and comfort with daily field travel
- Self-directed and accountable — you manage your own week against clear targets
- Comfortable with CRM-based work (HubSpot or similar)
- No clinical licensure required
Schedule: Monday–Friday, daytime field hours
