Home Care Outreach Specialist (Medicaid / OPWDD)
Familiarity with Medicaid, MLTCs, facilities, or OPWDD is a strong plus
Spanish-speaking strongly preferred
Field role only – digital/social media marketers should NOT apply
Experience in home care / healthcare outreach is a strong plus — but not required if you can prove you can produce results.
Compensation:
- Base salary + performance-based bonus
- Bonus structure tied to overall production and agency growth
- Top performers can earn $5K–$15K+ per month

This position is focused on building patient census in NYC, the Bronx, and Western Queens. Candidates should be familiar with these local areas.
We are looking to grow our patient base in these areas with experienced outreach professionals.
About the Role:
This is a field-based outreach position focused on generating patient referrals and building relationships.
What we’re actually looking for:
- Someone who is comfortable being in the field daily
- Strong communication skills and ability to approach new people
- Ability to build relationships and follow up consistently
- Self-motivated — no micromanaging
Experience in home care / healthcare outreach is a plus, but not required if you can prove you can produce
What you’ll actually be doing:
- Going into the field and building relationships with referral sources
- Speaking directly with MLTCs, discharge planners, social workers, and community organizations
- Generating new patient referrals for home care services
- Following up consistently to turn relationships into active cases
Additional:
- Spanish-speaking strongly preferred
- Must be comfortable with in-person outreach
This is a field/community outreach role. Digital marketers or social media marketers should NOT apply.
You will be expected to be out in the field multiple days per week, meeting new contacts and building referral sources. This is not a passive role.
If your background is primarily case management, intake, or administrative coordination (processing referrals), this is not the role. This position is strictly for generating new business.
