Duties
- Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to establish accurate diagnoses.
- Prescribe medications, manage treatment plans, and perform procedures such as suturing, IV infusions, and injections in accordance with state regulations and collaborative practice agreements.
- Utilize electronic health record (EHR) system for documentation, order entry, and medical records management.
- Provide acute care management for urgent conditions, including triage in outpatient or telehealth settings, and coordinate follow-up care.
- Collaborate closely with physicians, specialists in gastroenterology, infectious disease care, behavioral health providers, and other healthcare professionals to optimize patient outcomes.
- Engage in patient education on health coaching topics such as nutrition, chronic disease management, geriatrics, hospice & palliative medicine, and memory care.
Qualifications
- Valid licensure as a Nurse Practitioner or Physician Assistant with current certification to practice independently or under collaborative practice agreements.
- Proven experience working across multiple healthcare settings including hospital medicine, emergency medicine, ICU/critical care units, nursing homes, assisted living facilities or outpatient clinics.
- Familiarity with EMR/EHR systems such as Epic, Athenahealth, Cerner or eClinicalWorks; experience managing electronic health records efficiently.
- Strong knowledge of medical terminology, physiology, pharmacology including medication administration and CPT coding for billing purposes.
- Experience performing complex procedures like suturing, venipuncture (phlebotomy), catheter care/insertion, sterile processing techniques or tube feeding management preferred.
- Background in diverse areas such as geriatrics (memory/dementia care), behavioral health or occupational health enhances your versatility.
- Critical care experience including ICU or Level I/II trauma center exposure is highly valued; familiarity with infection control protocols is essential.
- Excellent client communication skills with the ability to educate patients effectively while maintaining HIPAA compliance; experience working with individuals with disabilities or developmental challenges is a plus.
