Responsibilities
- Dispense medications accurately while adhering to all safety protocols, including aseptic technique and compounding medications when necessary.
- Conduct thorough patient assessments to determine appropriate medication therapies, considering physiology, anatomy, medical history, and current health status.
- Collaborate with healthcare teams on medical management plans, including pain management, immunizations, and IV infusion therapies.
- Utilize pharmacy software and electronic health record (EHR) systems to process prescriptions, update patient records, and manage inventory efficiently.
- Provide expert patient service by counseling on medication administration, potential side effects, and drug interactions while ensuring compliance with HIPAA regulations.
- Manage inventory levels meticulously to prevent shortages or overstocking of medications while maintaining proper storage conditions.
- Perform specialized tasks such as IV insertion and sterile compounding for critical care or pediatric patients in hospital or LTC (Long-Term Care) pharmacy settings.
- Stay updated on current medical terminology, pharmacy practices, and regulatory standards to ensure compliance and optimal patient outcomes.
Experience
- Proven clinical pharmacy experience with a focus on acute care or hospital environments; LTC pharmacy experience is highly valued.
- Demonstrated expertise in critical care settings, including pain management and IV infusion therapies.
- Completion of a pharmacy residency program or equivalent clinical training emphasizing patient care and medical management.
- Experience working with pharmacy technician teams to coordinate medication preparation and distribution efficiently.
- Familiarity with EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) platforms for seamless documentation.
- Knowledge of immunizations administration and aseptic techniques essential for sterile compounding and IV therapy preparation.
- Strong understanding of physiology, anatomy, and medical terminology to support accurate patient assessment and treatment planning.
